I have reinstalled the game almost 3 times and using no mods.

If you re - installed, and you're not using mods it should be fine. Try checking where you got it from, it's probably a modified GTA if you got it by piracy.

I mistakenly added skybox files in my cleo folder. It's fine now.

How you manage to have cleo folder with clear install and no mods?

balla characters often spawn on my screen too, but they die and disappear really fast

I usually use modloader for 2dfx and cleo for steamemfix.
