goto Profile - Forum profile information - and there you'll find a table:
answering to your questions:
1) edit it in Custom Title line.
2) ranks are given automaticaly (it depends on posts quantity) or by admins.
3) edit it in Personal text line.

p.s. you can edit only your own profile.
