Reason of getting banned: for meh - you haven't changed one bit so you don't deserve the right to play
Further information: Please give me another chance. I am sorry for last time. I will not open my stupid mouth like that again. I know I said this last time also then got banned again. I really apologise.
I know I repeated one mistake two times but I'm sure I will not do the same again if you please give me one more chance. I will watch out what I am saying carefully. I promise not again. Please forgive me one last time.

Man I'm absolutely sold. This comment alone makes me want to unban you. Absolutely NOTHING there that would make me doubt your words.
On a serious note - either put some effort in to the request or we'll just close it.

Ok, I know I am untrustable now. Last time I said the same words and now I am here again with these same words. But they mean something more this time, compared to the last. I apologize again for all the offense. I know what I did wrong and what I should not do now.
After multiple warnings and kick, I didn't listen and just kept doing the same shit again. I shouldn't have insulted back in first place, and after even I did. I was warned by you, even kicked. I should have understood. I realize my mistakes already now. I will not do anything like this now, I will treat everyone with same respect, and follow the rules strictly. If someone rulebreaks I will report and not do same to them which they did. If you give me this chance I will take this very sincerely. All this time was enough for me to work on my mistake.

These words are supposed to mean something, but then I look at your history and I see you giving us promises left and right about how you'll start behaving, how you know the rules and yet you have created what, 11 unban requests?
You need to do better than all 10 previous requests combined because we're out of patience dealing with you and you're running out of chances real fast.

Yeah I know I have wasted a lot of chances and not much of them left now with me.
Making this better than all of previous ones is understandable because I made 10 appeals before this one, which is enough reason for you to not put your trust into my words.
After a lot of stupid appeal I got unbanned I told you you will not complain again but I kept doing mistakes.
But with 8 denied appeals I learn something every time. The last bans were for arguing with admins, I showed rude behaviour, I argued without even checking once who was wrong.
I also learned if you were wrong I should not do that kind of behaviour and insult someone's professionalism. You guys try to make server better and I make a nuisance when I come.
I'm sure it gets you pissed. I am sorry for behaving that way over and over again not only with admins but with players as well. I will not fall into same mistakes again this time.
I am shameful for my bad behaviour, I apologize to everyone. I did not mean to hurt anyone.
I will not get myself in trouble this time you will not have to be pissed because of me. I request this last chance I will control my tongue and behaviour this time. Please trust upon me, I really mean something.
