I have been using the RS5 V4 radar mod for a while. It is an HQ version of the GTA SA radar, with all of the locations labeled, and various types of properties/zones marked. I went to uninstall it since I just use modloader, when after I deleted it, in game, it was still there, but scrambled. The radar chunks were wildly out of place. I have replaced my gta3.img file, deleted my gta_sa.set file, completely reinstalled the game multiple times, tried installing other radar mods, but nothing has solved this issue. It is scrambled in singleplayer as well as SAMP.

In 0.3.7 R3, Kalcor adjusted the radar HUD to be more circular on widescreen, which I believe causes this. Try downgrading the client to R2, it'll still work on Argonath - https://files.sa-mp.com/sa-mp-0.3.7-R2-install.exe

Thanks for the advice. I will try it, and post an update in the form of a reply when I wake up in the morning.

UPDATE: No luck
I will be attempting a full reinstall as well, and then I will update again.

Just did a full reinstall, and the problem continues, except the radar mod thta was scrambled changed from the RS5 map to a different HQ radar, which I also do not have installed anymore. I have attached a screenshot of the issue.
This is on a fresh install, with no mods, except for samp 0.3.7 R2

No idea. That's insane if that's really a complete reinstall. Honestly at this point I would recommend deleting the entire GTA SA folder then do a reinstall because to me this indicates something didn't get deleted.

Thats what I did. I literally deleted my entire Rockstar Games folder, and my User Files folder in Documents. I remember I had a similar thing happen a while back, on a different computer, and deleting the mod fixed it, but this bug is ridiculous, and I can't even find any examples of anyone else experiencing it.

UPDATE: Just fixed it, thanks to a suggestion to use IMGTool to Rebuild the gta3.img archive. Keeping this here just incase someone else happens to have this issue.

For future reference, use modloader instead of IMG tool. Then it’s just drag and drop!

Whats interesting is that I was just using modloader, but even after doing complete reinstalls, and using a backup gta3.img, I was still having the issue. Somehow my mods made some sort of permenant change to my gta3.img file, and I basically had to download backup resources (weapon textures, radar textrues, etc) and rebuild my gta3.img file with them to get back to vanilla.

Looks like your "backup" gta3.img was fucked up in itself. Which is why the modloader was replacing the wrong radar files with the wrong data.
