Well i thought about this and the reasons i could think of are:

-Your computer is over heating
-You got some messed up mods
-Your video card is messing up
-Virus
-Its being overloaded by something

Does your screen go like:

If not could you post your computer specifications please? Then i can have a look and maybe figure out a possible solution.

Isn't that screen only at Windows 95/98/2000?

Well its not your graphic card that seems to be anything wrong with ... :pop:
If I were you, I would delete my whole GTA:SA Directory, and install/download it again.
If it's not that, then maybe you should try to install/remove Service pack 2/service pack 3 for XP - It has a effect on some games on some computers.

Screen Resolution:     1024 X 768 - 16 bit

This, you might have selected 32 bit colour in San Andreas, but your desktop resolution is 16 bit.
Try changing the colour on your desktop to 32 bits.

Why the hell do you have 16 bits colouring anyway?

You also set San Andreas to 32 bit? If yes, the only thing that might help is a complete reinstall as a few people mentioned.
This means, uninstalling SA:MP, then uninstalling San Andreas.

And also removing the 'GTA San Andreas' folder from your program files, and then just reinstall it.
Should do the trick :)
