i was banned for multi accounting and i am sorry for doing it. i was not aware that it was breaking a rule, and i am sorry. pleasee unban me

You should write:
-Your full nickname;
-Date and time on which you were banned;
-Who banned you;
-Reason for getting banned.

First you have to Resolve the unban request layout as Thomas pointed out
