So, I have recently had this problem, and still having it.. I searched on google, found some topics about solutions, but none of them worked.
My charger is plugged in, but not really charging my battery. I have tried uninstalling the software from Device Manager, and did "Scan for hardware changes", but still nothing worked.

I'm pretty sure my battery is not damaged, somehow. So I believe the problem is with the software, as most of users had it, and said over google topics.

I've got windows 8.1, in case that's important.

Have you tried using a different charger? might be a faulty one
try charging it in different positions it might work

If you're seeing this I'm not here

Does the laptop charge LED power? Does the charger LED power? Does the charger actually output anything?
How long does the laptop last without charger? Does the laptop shutdown immediately after plugging off the charger?

Most likely to be a fault charger like afmatt said. If it is indeed, then you have two options.
1) Replace the charger with the same official charger. (Expensive)
2) Replace the charger with universal charger & many outputs which always comes way cheaper than replacing the authentic one.
Something like this:

TheRock Kolta,
FBI Special Agent,
Los Santos Government

My laptop is working on the charger, so I assume it is not from the charger as well

I've got an original charger, and yes, it charges the battery sometimes, when i plug it in. But after few seconds, it shows "white LED", as if it is already charged.
My laptop without charger? It currently is on 6% battery power.. sometimes I turn my laptop to get it charged, and would need to actually remove the AC charger, then place it again so it starts charging, for a while.
Sometimes, this for-a-while does not appear, and charger happens to be charging for more than couple of minutes.

My laptop does not shut off immediately, but it actually goes on sleep mode, and i still can turn it on, but it will still go on sleep mode again without charger.

No, but I'll do so once you do that.

Sometimes I belive that the battery has been damaged or something else , try replacing the battery if that doesn't work , check the dc power jack , the place where you plugin the charger it might be damaged too.

I would think that too, if I had any problems when plugging in the charger.

The socket (power jack) needs repairing if a new battery doesn't solve it. Had the same problem a few years ago. I couldn't get my laptop to charge so I just left the charger in when I needed to use it, however after sometime the socket was heating up and started melting the charger (the head). So if you hear your laptop making any weird noise once you plug the charger in, I suggest you get the socket repaired asap as it's just going to damage your charger and your laptop even more.

This can be caused if the socket or cables are loose or damaged.
