I agree at some points,

Seeing as some boards remain a clusterfuck with all kinds of topics not being locked / removed or updated, forum moderators / admins could work on that.

Rarely do things need to be locked or removed.

I actually beg to differ on that one greatly. There are many non/cross-server boards that don't get moderated like they should quite often.

Just about all of them. There are many reports that go unanswered against flaming, provoking, trolling, etc., and many topics that users have created that have just gotten so far out of hand that they continue to wreak havoc until someone steps in days later.

I'm not really up for giving specific examples as pointing fingers isn't needed. We do need more people patrolling and handling reports though. People keep saying that there's only 4-5 reports a day and very little moderation needed, but I know that can't be right because I often make more reports than that in a single day!

Every topic dies on it's own accord. As for provoking, many here give it and cannot take it. Trolling is used far too much, by that I don't mean people are actually trolling, just the word itself is being used far too much and for pretty much anything and everything. I would probably be considered a troll as that is how much the word is being misused.

If you are making that many reports a day you may need to think 'Does the post I'm about to report really need reporting?' It may be that your reports are deemed unnecessary.

I'm fairly certain that when I see 2 pages of posts of people straight flaming and attacking one user and no one has moderated the topic that it's usually not an unnecessary report...

Sometimes people seem to be unsure on where to draw the line, so things can be confused/forgotten or administrated vaguely in certain cases.
A dedicated team could solve issues related to the above.
Often when it comes to reports, even those that can view/administrate them may "let them go", unsure on the best actions to take and/or unsure whether they should be the one managing it.

Punishements may also vary, depending on who issues them. A uniform system would be the most advantageous, and it would save the Owners, Community Leaders, Division Leaders etc. having to manage the majority of the things.

Another interesting 'tool' might be a section that allows users to report minor issues (perhaps not necessarily rulebreaking), move/lock requests etc. The report function could be made use of here, but in certain parts of the forum this could be counted as an out of purpose use for it.

I think the more these forums expand, the greater the number of users more the demand would be for this idea.

Edit: As I posted this and went to view the Forum and site ideas board almost immediately ran into 2-3 newer topics that should be in other boards. It would depend to what extend things would be administrated on, and how closely people are to keep to any forum guidelines.
It's good that we have a well rounded Argonath RPG Administration "unit". If a Forum Admin Team was required, a handful could more than likely be chosen from existing A-Teams.

4-5 months on now (time goes pretty quickly? o.O) and we've had some good advances.

It's very positive to see that the first foundation stones have been laid in restructuring the administration of these forums.
Obviously, it's a heavy task for the Owners and Community Leaders to do repeatedly and equally on all areas of the forum. Some people may have noticed the new 'Clans and Groups Moderators' - the concept is still in its early stages, but somewhat of a new Forum Admin Team is being put together. These C&G Moderators will obviously be supporting their relevant parts of the forums, but hopefully we're moving in the right sort of direction.
