Decreasing lag when playing IV/IV:MP

So.. I've recently bought GTA IV, and I was thinking about my specs.. I've tried SP and I experienced a bit lag while playing. But I didn't really care that much about it. (At least it didn't freeze and shits.)
But the thing is, how can I reduce the lag and get a little bit smoother gameplay (Not too much lag tho)

PC specs:

CPU: AMD Athlon(tm) II X2 250 processor (2 CPU's) On 3.0ghz, but I over-clocked it to 3.4 ( Yes, I made sure that it didn't over-heat etc.)

GPU: AMD Radeon 5450 HD. (1024mb)

Motherboard: N/A

RAM: Dunno what it is. But it's on 4GB tho.

That's everything I know about it, at the moment.

I guess I could take a look at the Topic Devin made to reduce lag in IV/SA etc. That could aswell help me.

Same CPU :P but I can play IV 20+FPS(NO MODS) 8400GS

No I don't play anymore
