Launch it from Launcher.exe ?

First of all , make sure you have GTA: Episodes From Liberty City installed in order to make Multiv work, then run Launcher.exe as adminstrator, it should work.

There are lots of IV Multiplayer clients, some guys are even developing one that supports peds and traffics.

Guys lets just say, one guy has a launcher which adds peds and traffic to the server and the other doesnt.So if one crashes to the traffic and he has the guy who doesn't has the same launcher(the one without peds etc.) then how will we know what happened?

This  case will never exist because the server will simply support only ONE type of client and not both.

Each client support it's own servers mate, it's like playing trying to play San Andreas MP from Vice City Multiplayer.
