Reporting a board moderator

Hi, I have created a format where you can report a board moderator if you think he mistreated you, is not fit for his job or is taking the wrong decisions. Remember you can not make up stuff, you have to stay polite in your report and at all times you have to tell the truth, if we find out you break any of the above stated items you might be punished on the forums.
Board moderators can not see these reports themselves, they'll be sent to a board only HQ can see.
