You need your game to be updated to patch 1.0.70 wich can be downloaded here.

Download the IV:MP installer,
=> www.gta-ivmp.com
You'll have to to indicate your GTA IV installation directory when installing, After installing IV:MP succesfully, You will find an application named LaunchIVMP.exe in your GTA directory, Launch it.
And that's all.

Install GTA 4
Install Patch 1.0.7 from   here
Install IVMP from   http://gta-ivmp.com/
