Big Bear Fat was used in Single Player, At the very end of the game, You see him at Glen Park while with Sweet. I think you save him or something but hes fat then
I remember him being skinny even in the end. I've completed the game several times on PC, and on PS2, and every time he made an appearence he was skinny.
Edit: After reading something on GTA Wiki, it's an unknown reason why there is an extra file for Big Bear with the same clothes, but just fatter, and it states even there that Big Bear is never seen fat. But, the most accurate prediction that was made is that before Big Bear started using drugs, and became a slave, he used to be fat, hence the name "Big Bear". And once using drugs, he became very skinny.
